A special tiara is an item dropped by one of the Suqahs found on Lunar Isle during the Lunar Diplomacy quest. This tiara belongs to Meteora, who lost it to the Suqahs. Meteora will exchange the Lunar amulet, if the player returns her tiara to her.
Despite the destroy text stating the Oneiromancer may provide another tiara, there is no dialogue branch that does this. Instead, the player will need to kill Suqahs to reobtain the tiara.
