The lever is a quest item in the Temple of Ikov quest. It is found in the eastern room in the Temple of Ikov after crossing the bridge which requires the player's weight to be below 0 kg (using boots of lightness or other weight-reducing clothing) to successfully cross. It is used to operate a broken lever which opens a door leading further into the temple.
After the quest, the item is still obtainable, but has no further use beyond the quest.
