The final letter is found by searching Keenan in the Wyrmscraig cathedral after defeating the Mad Angel during the Fallen From Grace quest. The letter is written by Elessia to the people of Wyrmscraig, informing them that she has left the island and left a golem in her place.

After all these years, I thought I had maybe finally mastered this illness. This hunger.
I was a fool. I have been a fool for so long now, and enough is finally enough. To the people of this island: know that you will be plagued no longer by my madness.
