The eye amulet is obtained during the quest Temple of the Eye. The player is requested to bring this item to the Mage of Zamorak in Varrock at the Altar of Zamorak to have him trace the object. The eye on the amulet is said to be the symbol of the Order of Wizards and gives off abyssal energy. The Mage of Zamorak will tell the player more about the amulet in exchange for a bucket of water and a Strong cup of tea.

Later in the quest, the player discovers from Wizard Persten that the amulet is related to a teleportation matrix and that a Transmission Spell has been cast upon the amulet, thereby allowing the Z.M.I to track the user of the amulet. In response to this, Wizard Persten alters the spell on the amulet to provide the Z.M.I with false information.

If the amulet is lost upon death, a new one can be acquired by speaking to Wizard Persten.

• The amulet appears identical to the blue Amulet of the Eye.
