The Balance Elemental is Guthix's guardian of the Stone of Jas, which he created to keep the Stone safe after the God Wars. It is encountered as the penultimate boss in While Guthix Sleeps in the Ancient Guthixian Temple as the player touches the Stone. The Elemental is given form by the four elements, and appears to be familiar with the player, knowing their name before having met them and apparently being aware of their past, present, and future.

The Balance Elemental has 750 hitpoints and attacks with all three combat styles. It follows a predictable pattern: a special drain attack, followed by four normal attacks, then repeats. Getting hit by the special attack without praying against it will result in the player's combat stats and prayer being drained. The first attack after each style change will always be a stat-lowering attack before returning to normal attacks. It also makes use of routefinding to avoid being safe-spotted in its melee form.

The appearance of its head determines its combat style. The correct overhead prayer must be active before the drain attack animation begins to negate it. Immediately switch prayers when the elemental changes appearance—you have approximately 2 ticks to react before taking damage. Note that when the head is white (airy), it's still a melee attack despite the projectile-like animation.

After reaching certain Hitpoint thresholds, it will begin to speed up its attacks and use protection prayers:

Players should be careful not to get overwhelmed by the fast attack speed and focus on keeping prayer up. Changing prayers to match the elemental's attacks should take priority over changing one's attack style. If the speed becomes overwhelming, use the pillars for cover while switching prayers and weapons, then heal or restore stats with a super restore or Menaphite remedy.

Upon death, a gravestone will be placed by Movario and Darve on the northern ledge of Chasm of Tears, where you initially attract the light creatures and enter the Ancient Guthixian Temple. The sapphire lantern is always kept on death, and it is not required to reach your gravestone to re-obtain items lost.
