Efaritay Hallow is the legendary Icyene queen who founded and ruled the kingdom of Hallowvale with her human husband Ascertes from their palace during the Third Age. With Ascertes, she had two half-human, half-icyenic children: a daughter, Larina, and a son, Safalaan. After 820 years of peace, a Zamorakian army led by the vampyre Lowerniel Drakan invaded Hallowvale. Ranis Drakan managed to capture Ascertes, which forced queen Efaritay to surrender immediately. It is unknown what happened to her after her capture.

In Sins of the Father, the Icyene Graveyard is visited, located south-east of Efaritay's former palace (now known as Ver Sinhaza). There, it is revealed by Vanescula Drakan that Efaritay is not buried in her family's tomb, suggesting that she could still be alive. It is also revealed that Safalaan Hallow is her son.

During The Blood Moon Rises, it is revealed Efaritay was held prisoner by Lowerniel Drakan, who used her blood to create portals to and from Vampyrium. She was rescued by Sugadinti Vitur, living with him and his disciples in the old stronghold of House Jovkai. She helps rescue the adventurer and members of the Myreque from Drakan, including her son. Throughout the quest, she assists in battle against Drakan and, when Sugadinti refuses to return to Vampyrium, works with the adventurer to return and save her son, who was subjected to the vampyrification process by Vanescula Drakan.

After the quest, she can be found in Castle Drakan where players can purchase the Sunspear, along with a variety of vampyrebane weapons that were used throughout the Myreque quest series. She can also provide players with another copy of their tome of experience should they lose or destroy it before redeeming their experience reward.

• In RuneScape 3, Efaritay remained imprisoned by Lord Drakan until after the adventurer and members of the Myreque killed Drakan during The Lord of Vampyrium, later appearing in River of Blood when a wyrd, the transformed version of Safalaan, appears. Similarly to Old School RuneScape, she sought ways to save Safalaan after his transformation into a wyrd.
• The appearance of the spear Efaritay wields during combat matches that of the reforged Sunspear from RuneScape 3, which she mentions was formerly her weapon during that game's quest River of Blood. There is it also revealed that the Wolfbane dagger originated from the bottom of said weapon.
• Efaritay was a renowned botanist, using her skills to create plant life to use against the vampyres, such as the blisterwood tree.^[1]
