An empty folder is a quest item used only in What Lies Below. It is given to the player by Rat Burgiss to store the paperwork that he asks the player to recover from outlaws in their camp west of Varrock palace.
As you add Rat's papers to it, it will become a used folder. When all five papers are added, it becomes a full folder. When handing in the full folder to Rat, any excess papers are also removed from the player's inventory.
